Elderly Care and Caring for Persons with Physical Disabilities Our Services include caring for: The Elderly and Frail, Dementia/Alzheimer’s Stroke, Parkinson’s, Multiple Sclerosis Spinal Injury, Paralysis, Cystic Fibrosis Cerebral Palsy, Muscular Dystrophy and          Palliative Care Additional Assistance We can provide information about, and help obtain, equipment from suppliers, and adapting homes with mobility aids. We work with multidisciplinary teams, including Social Services, Primary Care Trust, and National Health Service Professionals. The Service we provide consists of any or all of the following: Care for persons with learning difficulties, and other life-affecting conditions or behavioural problems Our Service includes caring for persons with: Learning difficulties; Autism and Aspergers Syndrome; Down’s and Lowe’s Syndromes; Epilepsy; Challenging behaviour; self-harming; Problems with drug or substance abuse. Supported Living We currently provide live-in support for young persons with physical and mental disabilities within a supported living environment in the community. The Service we provide consists of any or all of the following: 1. A person-centred, outcome-based approach 2. Assistance to achieve a good quality of life 3. Assistance to mobilise and in the use of mobility aids 4. Promoting independence, allowing for personal preferences 5. Knowledge of the use of aids for sensory impairment 6. Escorting on social activities, and overcoming obstacles 7. Budgeting assistance 8. Dietary needs monitored and encouraged 9. Incontinence pad changes 10. Medication, including appointments and repeat prescriptions 11. Personal care, observing dignity 12. Cooking and domestic work such as laundry, shopping, and cleaning 13. Support for day centre activities and hobbies 14. Supporting family and friends to work together 15. Reviews and multidisciplinary meetings 16. Documentation (such as complaints and quality assurance surveys) 17. Care plans and daily record keeping. Specialist Care Service with Training PEG Feed Palliative Care Stoma and Catheter Care Bowel Management 1. A person-centred, outcome-based approach 2. Assistance to achieve a good quality of life 3. Promoting independence 4. Understanding body language 5. Communicating using picture symbols 6. Escorting on social activities and overcoming obstacles 7. Budgeting assistance 8. Support a healthy diet incorporating and monitoring dietary needs. 9. Managing and assisting with incontinence. 10. Medication, including appointments and repeat prescriptions 11. Supervision of personal care 12. Prompting and support with cooking and domestic work such as laundry, shopping, cleaning 13. Accompanying day centre activities, hobbies and outings 14. Supporting family and friends to work together 15. Documentation (such as complaints and quality assurance surveys) with pictures and symbols 16.
